Dr. Awada's years of rigorous training began as a presidential scholar at Wayne State University where she completed a double major in chemistry and biology.
She graduated from Wayne State University School of Medicine with distinction and was inducted into the prestigious Alpha Omega Alpha National Medical Honor Society
in recognition of her demonstrated skill and commitment to excellence. Dr. Awada went on to become one of the first women to complete an integrated General and Plastic
Surgery Residency at the internationally renowned University of Texas Southwestern Medical Center in Dallas. There she trained with many of the preeminent plastic
surgeons in the world, mastering time tested techniques while developing new cutting-edge procedures.
As an authority in cosmetic surgery, Dr. Awada has been featured on CNN, MSNBC and FOX, discussing her breast enhancement and total body contouring techniques.
She has published several articles in distinguished medical journals and is a contributing author to the national Skin Deep magazine. As an educator, she serves as a
Clinical Instructor of Plastic Surgery and is a vocal advocate for patient education, safety, and satisfaction.

Dr. Awada employs over fifteen support staff, many of them with more than 10 years of medical healthcare experience. The center runs
efficiently because everyone knows their respective roles. That allows for a smoother, more enjoyable and consistent patient experience.

Our surgery center is accredited by the American Association of Accreditation of Ambulatory Surgical Facilities, Inc. (AAAASF).
AAAASF is a not-for-profit association devoted to a voluntary inspection and accreditation program for qualifying ambulatory plastic surgery facilities.
Accreditation by the AAAASF certifies that a facility has met the rigorous standards developed by the AAAASF for patient care, quality, and safety.
When planning surgery at any facility, you should ask if the facility is accredited. Accreditation can be an indication of the quality of the facility operation.
Michigan Cosmetic & Reconstructive Surgery has passed the strict guidelines for equipment, staff, hospital access, anesthesia administration and also requires that
the doctor performing surgery at the facility has privileges to perform the same procedures at an accredited hospital.

Although we would make every effort to assist walk-in clients, it is highly recommended that you schedule your appointment in advance to ensure that we can meet your
requests for a preferred date, time and treatment to accommodate all of your needs.

Confirmations
We will make a courtesy call to you at least 24-48 hours in advance of your appointment as a reminder, but we do ask that you assume the responsibility for remembering your appointments.
Cancellation Policy
If you are unable to keep your appointment time, please contact our office as soon as possible to reschedule your appointment.
We do request at least 24 hours advance notice for cancellations and you may be charged a $50 cancellation fee if you are unable to provide proper notice.
Minors and Children
Childcare services are not provided on site and due to
laser safety protocols, we ask that children do not accompany you to a scheduled appointment.

Aesthetician. After obtaining a thorough medical history, you will voice your specific concerns and desires you wish to achieve utilizing our state of the art Sciton Laser.
You will undergo a complimentary ($180.00 value) high resolution digital imaging automated skin evaluation utilizing the latest technology with the VISIA™ Skin Complexion
Analysis System. Upon completion of your VISIA™ complexion analysis you will be given the computer generated report and automated recommendations. Your aesthetician will
then provide you with a personalized skin analysis and discuss the most economical way to achieve your desires and address your concerns. You will discuss the recommended
personalized skin rejuvenation procedure benefits, risk, and cost and be assisted with financing if desired. Together, you will view our before and after photo gallery of
patients who have undergone the proposed laser treatment with your skin quality and concerns to foster realistic expectations. You will soon be on your way to a healthier,
younger, and rejuvenated new you!
Preparing For Your Consultation

Your first visit: Please bring a list of your current prescriptions, amounts and dosages. Be sure to include diet pills, aspirin, herbs and supplements.
No surgery will be performed on your first visit; the consultation is very important. Extra time is scheduled to allow for the required forms and for Dr. Awada
and her staff to evaluate your detailed history and perform an examination. It takes time to get to know you well enough to understand and evaluate your needs.
Then Dr. Awada will explain the options and procedures and answer your questions as we work out a program for you.
Due to the amount of information that we cover, bringing children usually distracts you from the full benefit of the consultation.
At your initial consultation another visit might be scheduled, giving you the additional opportunity to ask questions and clarify anything you may not understand.
Plan to be seen after your surgery. Postoperative visits (care directly related to surgery) are included in the surgical fee. Surgery is done on an outpatient basis,
and you can return to the comfort of your home once you are awake and stable. You MUST HAVE A RESPONSIBLE ADULT care for you the first night.
As you think about planning for the surgery, please think about who will care for you, how to take time away from your usual duties, travel plans, and bring your
appointment calendar. For example, with a breast augmentation, it takes about 4 weeks until you are able to lift 5- 10 pounds (even your infant can exceed that).

- THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) is a federal program that requires that all medical and dental records and other individually identifiable
health information used or disclosed by us in any form, whether electronically, on paper or orally, are kept properly confidential. This Act gives you, the patient, significant new
rights to understand and control how your health information is used. HIPAA provides penalties for covered entities that misuse Protected Health Information (PHI).
This Notice of Privacy Practices describes how we may use and disclose your Protected Health Information (PHI) to carry out treatment, payment or health care operations (TPO) and
for other purposes that are permitted or required by law. It also describes your rights to access and control your protected health information. "Protected health information" is
information about you, including demographic information, that may identify you and that relates to your past, present or future physical or mental health or condition and related

Uses and Disclosures of Protected Health Information
Your Protected Health Information may be used and disclosed by your dentist, our office staff and others outside of our office that are involved in your care and treatment for the
purpose of providing dental health care services to you, to pay your dental health care bills, to support the operation of the dental practice, and any other use required by law.
Treatment: We will use and disclose your Protected Health Information to provide, coordinate, or manage your dental health care and any related services.
This includes the coordination or management of your dental health care with a third party. For example, your protected health information may be provided to a dentist of physician
to whom you have been referred to ensure that the health care professional has the necessary information to diagnose or treat you.
Payment: Your protected health information will be used, as needed, to obtain payment for health care services. For example, obtaining approval for a hospital stay
may require that your relevant protected health information be disclosed to the health plan to obtain approval for the hospital admission.
Healthcare Operations: We may use or disclose, as-needed, your protected health information in order to support the business activities of your dentist’s practice.
These activities include, but are not limited to, quality assessment activities, employee review activities, and conducting or arranging for other business activities. We may use or
disclose, as needed, your protected health information to support the business activities of this practice. In addition, we may use a sign-in sheet at the registration desk where you
will be asked to sign your name and indicate your dentist. We may also call you by name in the waiting room when your dentist is ready to see you. We may use or disclose your protected
health information, as necessary, to contact you to remind you of your appointment. We may call your home and leave a message (either on an answering machine or with the person
answering the phone) to remind you of an upcoming appointment, the need to schedule a new appointment or to call our office. We may also mail a postcard reminder to your home address.
If you would prefer that we call or contact you at another telephone number or location, please let us know.
We may use or disclose your protected health information in the following situations without your authorization. These situations include: as Required By Law, Public Health

Your Rights
The Following is a statement of your rights with respect to your protected health information.
You have the right to inspect and copy your protected health information. Under federal law, however, you may not inspect or copy the following records; psychotherapy notes;
information compiled in reasonable anticipation of, or use in, a civil, criminal, or administrative action or proceeding, and protected health information that is subject to law that prohibits
access to protected health information.
You have the right to request a restriction of your health information. This means you may ask us not to use or disclose any part of your protected health information for the purposes of
treatment, payment or healthcare operations. You may also request that any part of your protected health information not be disclosed to family members or friends who may be involved in you
care or for notification purposes described in this Notice of Privacy Practices. Your request must state the specific restriction and to whom you want the restriction to apply.
Your dentist is not required to agree to a restriction you may request. If your physician believes it is in your best interest to permit use and disclosure of your protected health information,
your protected health information will not be restricted. You then have the right to use another Healthcare Professional.
You have the right to request to receive confidential communications from us by copy of this Notice from us, upon request, even if you have agreed to accept this Notice
alternatively (i.e. electronically).
You may have the right to have your physician amend your protected health information. If we deny your request for amendment, you have the right to file a statement of
disagreement with us and we may prepare a rebuttal to your statement and will provide you with a copy of any such rebuttal.
You have the right to receive an accounting of certain disclosures we have made, if any, of your protected health information.
We reserve the right to change the terms of this Notice and will inform you of any changes. You then have the right to object or withdraw as provided in this Notice.
Complaints
You may complain to us or to the Secretary of Health and Human Services if you believe your privacy rights have been violated by us. You may file a complaint with us by notifying our
privacy officer of your complaint at our office and main telephone number. We will not retaliate against you for filing a complaint.

Payment is due for all service when rendered. We accept Cash, Visa, MasterCard and Financing. We would be glad to assist you with applying for financing if desired.
